1976 - 2013
Age 37, of Maple Grove, passed unexpectedly on August 4th, 2013.
Survived by loving wife, Kelly; adoring children: Dominic, Ella & Faith; mother, Peggy (Mike); father, Randy; many other relatives & dear friends.
Visitation at Kozlak-Radulovich Maple Grove Chapel (13745 Reimer Drive) Thursday from 5-8 p.m. Funeral service at Lord of Life Lutheran Church (7401 Cty. Rd 101) Friday at 2 p.m. with visitation one hour prior. Interment Rush Creek Cemetery.
In lieu of flowers, memorials preferred to the family to be used for Jeremy's children's college fund.
